Our insurance program is underwritten by Horace Mann Insurance Company
Coverage for our members is as follows:

$1,000.00 of Life Insurance Coverage for all paid members, regardless
of age.

$2,000.00 of Accidental Death and Dismemberment Insurance Coverage for
all paid members, up to age 70

All claims must be accompanied by an ORIGINAL CERTIFIED DEATH
